Required Form and Fee Day for 2013-2014 Athletes

 

PLEASE NOTE: Hamilton High School’s Athletic Department has a mandatory Form and Fee day for fall athletes. This includes FOOTBALL, GIRLS AND BOYS CROSS COUNTRY, CHEERLEADING, POMS, BOYS SOCCER, GIRLS SWIMMING/DIVING, GIRLS TENNIS, GIRLS VOLLEYBALL and GIRLS GOLF.

 

Parents or athletes must present all required forms and the fee on one of the following dates:

Monday, July 15, 7-10 am                       
Tuesday, July 16, 3-6 pm

The following forms are required for all student athletes to practice - NO EXCEPTIONS:

1a. WIAA PHYSICAL FORM (green) - required for all freshman, new HHS students and those with expired physicals* - requires doctor and parent signatures   OR

1b. WIAA ALTERNATE YEAR PHYSICAL FORM (brown) – required for those who have a current physical on file – requires parent signature 

2. EMERGENCY CARD (yellow) – requires parent signature 

3. CONCUSSION FORM (blue) – requires parent and student signatures 

4. COCURRICULAR AGREEMENT (white) – requires parent and student signatures 

5. PARENT/PLAYER VIDEO (online) 

6. USER FEE - $30, cash or check. Checks should be made payable to HHS. Students who are cut from a sport will have their fees refunded; all other participant fees are non-refundable for any reason, including ineligibility or quitting.

 *Per WIAA rules, physicals taken after April 1 are good for the following two school years. Physicals before April 1 are good for the current year and the following year only

** Forms and fees will not be accepted prior to July 15th

** Student eligibility will be recorded and sent to coaches upon completion of ALL forms, fee submission and video verification

**All forms are available for viewing and printing on the Hamilton High School website; forms are also available outside Office C between 7 a.m. and 3 p.m. (http://chargers.hamilton.k12.wi.us/forms)

 1. Paperwork for latecomers will only be accepted from 1:00-3:00 p.m. between July 29th and August 3rd

2. Paperwork will only be accepted in Office C (Athletics and Activities office).Please note the following start dates for each sport:

Hamilton is now able to notify you of and sports cancellations and schedule changes via text
and e-mail. Go to www.hamilton.k12.wi.us > Hamilton High School > Athletics and Activities >
Schedules > Notify Me (in the upper right corner) and follow the instructions. By adding your
e-mail or cell phone, you will be immediately updated of a sport cancellation or schedule

 NFHS offers free online course for parents

The National Federation of State High School Associations has produced a free online course,
"The Role of the Parent in Sports." This free course is designed to educate parents about their
role as a sport parent in the interscholastic setting. The NFHS developed a 10-minute introduction
to the course that can be shown at preseason parent meetings.

This introduction provides a course overview and assessment of parents' understanding of their
role as they interact with their child, coaches, officials and other parents.
